   Physical Fitness Assessment Form (Collected at fitness test)
   Cover Sheet Checklist (Collected at entrance exam)
   Frontline Copyright and Nondisclosure form (Sign second page, Collected at entrance exam)
   
   (The Physical Fitness Assessment Form should not be completed any earlier than 30 days before the test)
   
   You may also request a hiring packet be mailed to you.
2. Sign-up for the Physical Fitness testing and Entrance Exam, contact Shaun Crowell in Human Resources.   
   
3. Bring your Physical Fitness Assessment Form to the Physical Fitness testing at Fargo South High School Sports at 2001 17th Ave. S.
   
   
4. Bring your Background Investigation packet and payment ($25.00 payable to the Fargo Police Department) for testing to the entrance exam, which is at the the Skills & Technology Training Center, 1305 19th Ave. N. Fargo, ND 58102
